The deadline to opt-out for FY2022 has passed. To opt out for the July 2023 through July 2024 cycle please print and complete the form below. Tape the completed form to the inside of the lid of your town-issued cart, and return the cart to 629 Randolph Avenue, between 8:30 am to 3:30 pm Monday through Friday, not later than June 30, 2023.
Please ensure the interior of your cart is empty, clean and dry before returning. When returning your cart, leave the cart in the area in the parking lot just in front of the main office. If you are unable to transport the cart to 629 Randolph Avenue, call 617-898-4968. If you don't have access to a printer, please hand write the required information and follow the instructions above.
